Medical Reimbursement Form
SPSU has implemented a new process for medical reimbursement. All currently enrolled SPSU students are eligible for a maximum reimbursement of $100 for expenses incurred at the Wellstar Powers Ferry Urgent Care.
Now, when using the urgent care, students will be expected to pay or use your medical insurance at the time of service. To be reimbursed for the money spent there, students must bring your receipt, ID, and medical reimbursement form to the Nurse's office in the Recreation and Wellness Center.
**You must turn in your medical reimbursement form and your receipt, during the semester the charges were incurred.**
The medical reimbursement form is available in the nurse's office, front desk of recreation center, Powers Ferry urgent care, and online at Student Health Services.
Online and Georgia Highlands students are not eligible for the $100 health credit.
